Thank you for your referral. Guiding Light Psychiatry will reach out to your patient directly to arrange their appointment.

Please download the referral form below and fax it to our office. Our team will make up to three attempts to contact your patient by phone and/or text to schedule their visit. If we are unable to reach the patient, we will securely store their medical records and notify your office.                                                                        The patient may then contact us when they are ready to schedule.
