Thank you for your referral. Guiding Light Psychiatry will contact your patient directly to schedule their appointment.

You may download the referral form below and please fax to our office. We will fax a confirmation of your patient’s appointment date and time after it has been scheduled. We will make three (3) attempts to contact the patient to schedule an appointment (either via a phone call and/or text.) If we are unable to schedule the patient, we will scan in their medical records so they may contact the office when they are ready to schedule an appointment, and notify your office via a text notification.  If you have any questions, please contact our scheduling department at Help@guidinglightalabama.org
