All Intake Form fields are required; services are available for patients 16 and older. If you are experiencing a mental health emergency, call 911. After you submit your Intake Form, we will verify your insurance, review your information, and contact you to schedule your appointment; due to the high volume of new patient inquiries, your copay will be collected at the time of scheduling to reserve your appointment time and applied to your first visit. This pre-payment is non‑refundable for no‑shows or cancellations made with less than 24 hours’ notice. Please allow 7–14 days to receive a call to schedule your appointment. For self‑pay patients, the initial visit is $200 with a non‑refundable $100 deposit required to secure the appointment, and follow‑up visits are $120, due at the time of service.
