New patient appointment request

If you are experiencing a mental health emergency, call 911 or go to your nearest emergency department immediately. To become a new patient, submit an appointment request. Our New Patient Intake Team will review your request, verify your insurance, and determine whether we have a provider who is the best fit for your needs. We strive to schedule new patient appointments within 7–14 days whenever possible. Due to the high demand for appointments, all new patient evaluations are reserved exclusively for you and require a valid debit or credit card on file. A $75 fee will be charged for missed appointments or cancellations. Because we reserve a full hour for each new patient evaluation, we ask that you attend your scheduled appointment or provide at least 24 hours' notice if you need to reschedule. This policy helps us provide timely access to care for all patients.
